    • The registration form must be submitted no later than 11:59 p.m. by the deadlines indicated below in red.
    • Families will receive an email approximately two weeks prior to the testing window indicating where your child will test, along with the testing date and time. This email will be sent to the email address listed when registering.

     Testing Locations:

    • We provide two testing locations based on zip code: R.L. Brown Elementary and the Schultz Center. Students who live in the north and west part of Duval County will test at R.L. Brown Elementary. Students living in the south and east part of Duval County will test at the Schultz Center. At times, we schedule students at the Parent Resource Center.

      • Schultz Center, 4019 Boulevard Center Drive, Jacksonville, 32207

Please click here for the Assessment Services Student Identification form. The Assessment Services Student Identification form will also be attached to the State Testing Information email you will receive two weeks before the start of the testing window. This email will include your exact testing date, time and location.
    • Students must adhere to the С³óߣ dress code while at the testing site.
    • Handheld calculators are not permitted, except for students who have this accommodation on their IEP or 504 Plan. Students taking assessments that allow a calculator will use the online calculator provided within the testing platform.

Families with multiple children testing will need to complete a form for each child.  Do not email any completed forms to the State Testing Office as it contains confidential information and will be deleted.
